Presenter Attestation
According to guidelines set forth by the Accreditation Council for Continuing Medical Education (ACCME), should it be determined that a conflict of interest (COI) exists as a result of a financial relationship, this conflict must be resolved prior to an educational activity. If you have disclosed a potential conflict of interest that may affect your presentation, you MUST fill out an attestation form. Attestation forms are due to ckissinger@srs.org on June 14, 2011.

Dual Podia
Presenters will alternate between two podia, placed on opposite sides of the stage. Please arrive early to your session and be ready to take the podium opposite the presenter before you BEFORE that presenter has finished speaking, to facilitate quick transitions.
Confirmation & Registration
All podium presenters must confirm their plans to participate by registering for the program by May 1, 2011. Please visit the Registration tab to register online (note that ICL faculty are automatically registered for the meeting). It is not necessary to enter the abstract ID number during your registration this year.
Program Book Publication
Your abstract will be published in the scientific program book as submitted. If you need to make any corrections to the title or author listing, please send them by May 1 via e-mail to meetings@srs.org. We cannot make edits or additions to the abstract text. **Also, authors may only be added to an abstract if they have an existing author account in the abstract submission system and a completed COI Disclosure Form. Sorry, no exceptions.
Cancellation & No-Show Policy
If you are unable to present this paper in person, please inform us ASAP which co-author will present in your place. If no one is able to present this paper, please inform us by May 1 so that we can substitute an alternate paper. In the case of a “no-show” podium presentation, please note that all authors & co-authors associated with the “no-show” paper will be prohibited from presenting any papers at the next two IMAST meetings.
Non-Exclusive License and Manuscripts
Both a Non-Exclusive License Form and a complete manuscript are required from all podium presenters no later than June 1, 2011. The Non-Exclusive License Form and guidelines for the manuscript may be downloaded above.
Advance Submission
Advance submission of your paper presentation is REQUIRED by July 1, as ACCME guidelines now require that CME-providers review presentation content prior to the educational activity. Please note that paper presentations will be reviewed in advance of the meetings by the SRS CME Committee and that presenters will be REQUIRED to pre-upload their PowerPoint presentations to the Online Speaker Ready Room no later than July 1, 2011 to facilitate this review. Please prepare yourselves for this change and allow time to complete your presentation in advance.
Presentations maybe submitted to the Online Speaker Ready Room, using the link above.
The presentation upload site for podium presentations will close at 5:00pm ET on July 1. Per the advance submission requirement, all presentations must be submitted by 5:00pm ET on July 1 and presentations will not be accepted onsite.
Disclosures
Please note that an additional slide including all authors’ financial disclosures will be inserted at the beginning of your talk for brief display at the start of your presentation. Financial disclosures were collected at the time of abstract submission. If you have any corrections to make to your disclosure statement, please send them to meetings@srs.org.
Housing & Travel
Presenters are expected to register, pay required registration fees and will incur all costs for housing and travel. Please visit www.srs.org for details on arranging your housing and travel for Copenhagen.
