Registration Procedure
Online registration for the 47th Annual Meeting is encouraged. However, delegates may also register by completing and mailing or faxing the PDF Registration Form. All registrations will be confirmed via email. If you do not receive a confirmation within two weeks of submission, please contact the Scoliosis Research Society at +1-414-289-9107, or meetings@srs.org.
To register for the Pre-Meeting Course ONLY please fill out the Registration Form and return it to the SRS office by email or fax.

What’s Included in the Delegate Base Registration Fee?
The delegate base registration fee includes entrance to the General and Concurrent Scientific Sessions, Lunchtime Symposia, breakfasts, breaks, lunches, and the Welcome Reception on Wednesday, September 5.
Additions to the Delegate Base Registration
The following items may be added to the base registration for an additional fee. It is recommended these items be added during initial registration, as delegates will not be able to edit their registration online at a later time.

Benefits of Advance Registration
There are many benefits to registering in advance for the Annual Meeting including:
- Guaranteed materials pick up upon arrival in Chicago
- Shorter lines that move quickly
- Guaranteed access to social events such as the Farewell Reception
- Significantly lower registration rates than those charged onsite
- Onsite access to online evaluations and CME certificates
Guest Attendance
Guests must complete and submit a separate registration form. Guest registration fees include access to the Guest Hospitality Suite which includes breakfast from Thursday, September 6 through Saturday, September 8, entrance to the Welcome Reception on Wednesday, September 5 and the ability to register for social events including optional tours and the Farewell Reception, for additional fees. An additional $25 charge applies for each ticket to the Farewell Reception on Friday, September 7. Substitutions
Name substitutions must be made to SRS in writing prior to August 15, 2012. Any name substitutions made onsite will result in a $100 processing fee. If the original registrant shows up at the meeting, he/she will be responsible for a new onsite registration fee.
Cancellations & Refunds
All cancellations must be received in writing. Delegates may fax their cancellation to (414) 276-3349 or email it to meetings@srs.org. Delegates will receive a confirmation and refund within 14 days of receipt of their cancellation notice. SRS will grant a full refund, less a 10% processing fee, for cancellations received in writing on or before August 20, 2012. No refunds will be granted after August 20, 2012.
