All Patient Outcome Questionnaires found below are free for anyone to use. No permissions or licensing is required. Users are welcome to create additional translations. If translated to a new language, SRS asks that you share the new translation for inclusion on this site.


SRS has removed the SRS-23, SRS-24, and SRS-30 from the SRS website. All practitioners should utilize the SRS-22r, and its various translations, as it is the most recent and most validated version of the SRS Patient Based Outcomes Questionnaire. The historical versions of the questionnaire are cited in the bibliography and can be referenced in that manner.

1. What are the age ranges for the SRS-22r?

The SRS-22r has been validated in children ages 10 and up. It is also suitable for adults. There is a single version of the SRS-22r usable for both children and adults.

2. Are children supposed to complete the questionnaire independently? Or can they have help from parents or care givers?

Children 10 and up should be able to complete the questionnaire independently. Parents/caregivers can assist but should not complete it for them.

3. How was validity tested?

The Outcomes were developed to assess patient satisfaction with respect to the treatment of scoliosis. The instruments test for reliability, validity and sensitivity. For further validity tests we recommend doing a Medline search to find abstracts that utilized the Outcome surveys.

4. How many missed/removed questions are allowed before the entire questionnaire is considered invalid?

SRS advises that if there are more than 4 missed questions it would be considered invalid.

5. Who can use the SRS Outcome Questionnaires?

All Patient Outcome Questionnaires are free for anyone to use. No permissions or licensing is required.

6. What languages are available in the SRS-22r?

All provided translations are available on our website.

7. I have questions about the LD-SRS (Limb Deformity) tool

The "LD-SRS" is not an SRS produced tool; outside physicians modified the SRS-30 to use as their own instrument. Therefore, we cannot provide any information on this tool.

8. What does the “r” stand for in SRS-22r?

The “r” stands for refined indicating the most thoroughly validated version of the Outcome questionnaire.

9. I am using a different SRS tool (SRS-23, SRS-24, and SRS-30) should I switch to the SRS-22r?

SRS recommends all practitioners should utilize the SRS-22r, and its various translations, as it is the most recent and most validated version.
